For all other EU countries
Please provide the following personal information:
• Full name
• Date of birth
• Home address
• Telephone number and email address
• European Health Insurance Card/EHIC (if applicable)Please also provide your medical information:
• A referral letter from your general practitioner or treating specialist requesting a second opinion
• A summary of your medical history and current diagnosis
• Relevant radiology reports and laboratory results
• A pathology report (if available)It is advisable to contact your health insurance provider prior to requesting a second opinion. They can inform you about the appropriate procedures and requirements. In many cases, an S2/E112 form is necessary in order to receive reimbursement from your insurer. If you do not have health insurance, or if you reside outside the European Union, we advise you to consult any applicable insurance provider before requesting a second opinion. They may be able to inform you whether (partial) reimbursement is possible. In some cases, a warranty statement is required.

Below you will find the steps for requesting a second opinion at the Brain Tumor Expertise Center, Maastricht UMC+.
Step 1:
Contact your treating physician or general practitioner. Ask for a referral letter for a second opinion.Step 2:
Preferably contact your health insurance provider. Ask about coverage and required documents (for example, an S2/E112 form or a guarantee statement).Step 3:
Submit all required documents. Send your personal details and medical information by e-mail to:poli.oncologie@mumc.nl, Attn. Dr. Broen / Dr. Hoeben
Subject line: Second Opinion – Neuro-OncologyStep 4:
Medical file review. Once all documents are received, a medical specialist will review your file. You will then be contacted by phone or e-mail to schedule an appointment at the outpatient clinic.Step 5:
Multidisciplinary consultation. Your case will be discussed within the oncology team in order to provide the best possible advice or treatment plan.Step 6:
Follow-up consultation and feedback. You will be invited again to the outpatient clinic to discuss the advice. A summary will also be sent to your referring physician.Questions?
